			<content:encoded><![CDATA[<p>If you’re considering <strong>breast augmentation</strong>, you’ve undoubtedly been wondering whether you should have your implants placed over or under the muscle.</p>
<p>In my previous blog, I discussed the pros and cons of over the muscle breast augmentation.  Below, I will discuss under the muscle breast augmentation.</p>
<p>One of the determining factors in deciding where to place a breast implant is the shape desired by the patient.  The relationship of the implant to the pectoralis muscle causes varying degrees of shapes.</p>
<p><strong>Advantages </strong>of placing a breast implant completely behind the muscle include a more natural breast shape (as opposed to a rounder shape achieved by placing the implant over the muscle), low interference with mammograms, little to no implant visibility, a reduced risk of capsule contracture, little to no rippling, and no visible scars on the breast (the incisions are made at the anterior axillary fold- under the breast).</p>
<p><strong>Disadvantages </strong>of under the muscle breast augmentation include slightly more post-op discomfort and difficulty finding a surgeon completely competent in this procedure.  Not all cosmetic surgeons have mastered this delicate skill of placing the implant totally behind the pectoralis muscle of the chest wall.</p>
<p>Because of the natural shape achieved and the reduction in interference with mammograms, this is a preferred method of breast augmentation of many cosmetic surgeons.</p>
<p>As with any surgical procedure, careful thought and consideration must be placed on your lifestyle, level of activity and aesthetic preferences.  Be sure to thoroughly review all of your options with your plastic surgeon.</p>

<p><strong>A:</strong> <strong>A</strong> <strong>mid facelift</strong> is also known as a <strong>cheek lift</strong>.  <span>Other names</span>: Midface suspension, SOOF lift (a less comprehensive but related procedure), meloplication. It focuses on improving the area around the cheeks when there is no sagging in the neck.  It lifts the <strong>lower eyelid</strong> to the upper lift. If a patient has lines around the mouth also known as <strong>marionette line</strong>, mid facelift can correct this. It can also help smooth out the area under the lower brow and restore fat in the cheeks so they down sag.   A facelift is a combination of a face  lift and a brow lift.</p>
<p><strong><span>Advantages</span>:</strong> Midface lift may produce a more natural appearance (less pulled) around the cheek and mouth than with a &#8220;classical&#8221; face lift, which pulls tissue more towards the ear.</p>

<p><strong> </strong></p>
<p><strong>Q</strong>: Rather than get a face lift I would prefer a nonsurgical treatment. What do the doctors say works best for sag in jowls and loose neck skin?&#8221;</p>
<p><strong>A: There is no substitute for a face lift. </strong></p>
<p>Depending on the tone and condition of  the patient&#8217;s skin and age a total face lift may not be required.</p>
<p>In the thirties to mid forties to reduce and fill wrinkles fillers like<strong> Botox,  Restylane</strong>, and <strong>Radiesse</strong>, can be used. <strong>Laser resurfacing</strong> is also a way to smooth the skin and reduce lines.  In the mid to late forties and beyond fillers will most likely not be enough to reduce significant folds or lines in the skin.</p>
<p>For significant sagging jowls, deep wrinkles, and loose neck skin a surgical lower lift is recommended.</p>
<p>Most patients who decide to have this surgery are age forty five to fifty five.</p>
<p>During a lower face lift, underlying connective and muscle tissue known as the SMAS are lifted, the facial skin is tightened and the excess skin is removed.  The procedure usually takes about three hours and can be performed under general or local anesthesia.</p>
<p>Patients are back  to work in the swing of their normal activities in one to two weeks.<br />
Your doctor may also suggest a neck lift, a forehead lift or a mid face lift if needed.</p>

<p>Silicone implants come prefilled from the manufacturer with cohesive gel silicone.</p>
<p>Silicone implants require a larger incision and cannot be adjusted in volume intra-operatively.</p>

Television has provided the public with a glimpse of what a person goes through before and after their procedures.  Most people who have had work done agree that it was worth it.  Dr. Lombardi’s goal is to provide a safe, nurturing confidential environment where patients are free to ask questions and are well cared for. <p>How many of these procedures have you personally performed?</p>

<p><strong>Know your Doctor</strong>:</p>
<p>Anthony S. Lombardi, M.D., F.A.C.S., P.C., who is board certified in plastic and reconstructive surgery as well as general surgery, performs laser rejuvenation techniques, including botox and other wrinkle reduction therapies, microdermabrasion, hair removal, vein removal, pigmentation reduction, and acne treatments, including laser treatments, topical and oral medications, vitamin therapy, and nutritional counseling.</p>
<p>Dr. Lombardi is a member of the American College of Surgeons and the American Society of Plastic Surgeons.<br />
Dr. Lombardi received his medical degree from the University of Medicine and Dentistry of New Jersey and completed his residency in general surgery at Monmouth Medical Center in Long Branch, NJ.</p>
<p>He also completed a residency in plastic and reconstructive surgery at the University of Medicine and Dentistry of New Jersey.</p>
<p>Dr. Lombardi frequently lectures on the psycho-social aspects of plastic surgery, as well as on body dismorphic disorders.</p>
